Pre-Admission & Admission Requirements

For a planned admission, your doctor will advise the probable date of hospitalization.

You must apply to your TPA for pre-authorization at least 4–5 days in advance for estimated expenses.

If immediate hospitalization is required, our Corporate Help Desk will assist you with the process.

Please note: The Corporate Help Desk only facilitates the process and cannot influence TPA decisions.

Possible Reasons for TPA Pre-authorization Rejection

The illness/treatment is not covered under your policy.

Information provided in the pre-authorization form is incorrect or insufficient.

Your Sum Insured for the year is already exhausted.

Pre-authorization Process – Step-by-Step Guide

Step 1:

Contact the Corporate Help Desk

Upon arrival at the hospital, visit the Corporate/TPA Help Desk.

Step 2:

Present Your Health Insurance Card

Submit the original health insurance/TPA card issued to you.

Step 3:

Collect the Pre-authorization Form

The Help Desk will provide the appropriate form related to your TPA.

Step 4:

Fill the Two Sections of the Form

(A) Policy & Personal Details:

This section must be filled by the patient/relative.

The Help Desk will assist if needed.

(B) Treatment Details:

This section must be filled and signed only by the treating doctor.

Do not attempt to fill this part yourself.

Step 5:

Submit the Form for Verification

The Help Desk will check the form for completeness and inform you of any corrections required.

Step 6:

Submission to the TPA Office

Once complete, the form will be sent to your TPA via fax/email.

Step 7:

Approval / Denial Update

The Help Desk will inform you about the TPA’s approval or rejection status.

Step 8:

Deposit at the Time of Admission

A deposit amount may be required during admission.

This will be refunded after discharge once all formalities are completed.
